Create a Form In Admin

Go to "Form Builder" by navigate menu inside “Form Builder > Forms” and click on it.

CONFIGURE EXTENSION AS PER YOUR STORE

Go to Form Builder > Forms>, click on add button to create the new form or edit existing form.
